Yes. Children under 16 always apply in person on DS-11, so you pay $35.00 plus $100.00 for a book.
Under 16 is always in person
There is no DS-82 for a minor. Even if the child had a book as a baby and it is about to expire, you start over on DS-11. Both parents or guardians usually appear with the child. If one cannot, Form DS-3053 plus a notarized copy of that parent’s ID is the usual workaround. A solo parent with sole custody brings the custody papers instead of the other adult.
Infants need their own book. They cannot ride on a parent’s passport. Photo rules are awkward for newborns who cannot sit; studios that do infant passports know the white-background, eyes-open expectation. See newborn passport notes and photo requirements.

Book application $100.00. Card application $15.00. Execution once per appointment. Expedite $60.00 if you cannot wait 6-8 weeks. Photo estimate $15.00 if you do not already have a print.
The $135.00 book total is not a baby discount on the adult application. It is a different fee line on the official schedule. Do not mail an adult DS-82 with the child’s name in it.
What to bring besides money
Child’s evidence of citizenship, parental IDs, relationship evidence (often the birth certificate listing both parents), the photo, and the two payments. Facilities will turn you away if the child is at home. More on the documents page.
